Low back pain is a very common problem and a significant cause of morbidity and socioeconomic loss. It is the second leading cause of disability and poor activity. It imposes a considerable amount of pressure on society and the NHS, and is the most expensive musculoskeletal disorders.

Low back pain can also be associated with leg pain, many factors contribute to this process. These include disc degeneration or herniation, spinal facet joint degeneration, ligament inflammation or growth, ligamentous ossification, congenitally narrowing, ligament laxity, and subluxation. Advancing age ultimately contributes to these pathological processes.

The good news is, the majority of patients with lower back pain can fortunately be managed conservatively with a combination of therapy and pharmacological drugs, without the need for surgical or invasive procedures. Conservative treatment includes relative rest, structured exercise, physical rehabilitation therapy, manual therapy and pain medications such as nonsteroidal anti-inflammatory drugs (NSAIDs). Should these fail, lumbar spine or epidural injections can be administered for pain management. Surgery is indicated, in cases that do not respond to pain management or if neurological deficits develop.

New biological therapies have been and are continuing to be investigated for use of chronic low back pain treatment. These biological therapies in regenerative medicine so far have showed a very good promising success. One such treatment is the use of platelet-rich plasma (PRP). PRP is plasma containing concentrated platelets acquired from blood. PRP he’s also thought to be a bridge between conservative therapy and surgery. Platelets are rich in healing cells and growth factors, and are used in clinical practice to accelerate the body’s repair processes. They could, therefore, be beneficial for treating chronic low back pain associated to progressive degenerative changes and inflamed nerves.

A Randomized Double-Blind Controlled Pilot Study Comparing Leucocyte-Rich Platelet-Rich Plasma and Corticosteroid in Caudal Epidural Injection for Complex Chronic Degenerative Spinal Pain (2020)
 

The above study looked to compare the efficacy and safety between leucocyte-rich platelet-rich plasma (LR-PRP) and corticosteroid in guided epidural injection for patients with complex chronic lumbar spinal pain.

Both PRP and corticosteroid for epidural injections under guidance are equally safe and therapeutically effective in patients with complex chronic lumbar spinal pain. However, PRP is superior to corticosteroid for a longer pain-relieving effect and improvement in quality of life.

The results of guided PRP injections are similar to steroid for the treatment of persistent pain. However, long-term results are better in the PRP. group, and could be a safer option.
