Musculoskeletal Conditions & Treatment for Pain and Injury Solihull

Musculoskeletal Conditions

WHERE IS MY PAIN?

In this section, we will discuss some of the varying different types of pain and conditions one may have. Often an injury is often overlooked, but it is of fundamental importance that the whole body is assessed to distinguish likely possibilities of predisposing factors.

DISC INFLAMMATION (ANNULAR TEAR)

Disc bulges are slight protrusions of the nucleus into the deeper fibres of the annuals, causes a budge. Although it is typically little or no pain associated with a bulging disc, the affected disc may eventually suffer a protrusion or herniation - which is when the nucleus pushes through the damaged shell and cause pain and further complications. Bulging discs may also place pressure on nearby nerves, leading to serious discomfort and, in some cases, severe and chronic pain.

Our spinal discs are strong ligaments that connect our vertebral segments. The discs allow for mobility of the spine and shock-absorption. Each disc has a strong outer layer called the annulus, and a soft, jelly-like centre called the nucleus. The strong annular fibres contain the nucleus and distribute pressure evenly across the disc.

LOWER BACK

Commonly referred to as a 'slipped' (prolapsed) disc, BUT please note the disc does NOT actually slip. What happens is that the nucleus bulges too far out (herniates) through a weakness in the outer part of the disc (annulus). A prolapsed disc is sometimes called a herniated disc. The prolapsed disc can be extremely painful due to huge amounts of local inflammation and muscle spasms. It can also press on nearby structures such as a nerve coming from the spinal cord. Local inflammation may irritate a nerve also and cause local nerve root swelling.

Sciatica is pain and tingling that is felt down the leg that results from irritation of the sciatic nerve. The pain of sciatica is typically felt from the low back, through and deep to the, to behind the thigh and can radiate down below the knee. Often altered sensation or pins and needles can be felt in the calf or foot. The sciatic nerve is the largest nerve in the body and begins from nerve roots that exit from the lower back and extend through the buttock area to send nerve endings down the lower limb.

Dysfunction in the sacroiliac (SI) joint often causes lower back and/or leg pain. Leg pain from the sacroiliac joint can be particularly difficult to differentiate from radiating leg pain caused by a lumbar disc herniation(sciatica) as they can feel quite similar, however, with sacroiliac pain there seems to be a particular pattern distribution of referred pain.

Each segment of your spine has two joints either side, these are the facet joints. These joint register and allow for movement. The facet joints are subject to constant, repetitive motion, and can become worn or irritated as a result. Also, as the discs begin to reduce in size through normal age-related degenerative changes, this then also puts added stresses and strains on our facet joints. Reduced disc high causes increased facet compression. This can then result in friction, that then often results in irritation, inflammation, pain, and facet degenerative changes over time.

SHOULDER

The rotator cuff is a group of 4 important muscles that connect the arm bone to the shoulder ball at the ball and socket. The rotator cuff allows for dynamic mobility at the shoulder, allowing for high amounts of mobility, whilst at the same time allowing for stability of the joint. The tendons run under part of the shoulder blade, at the top of the shoulder, and this where they are very vulnerable to being pinched and damaged. overtime repeated pinching can lead to tearing of the rotator cuff muscles and/or tendons, resulting in pain and weakness.

Shoulder arthritis can be extremely painful and make everyday activities such as lifting a heavy dish into a cupboard, hanging clothes on a washing line, or even steering a car difficult. The shoulder is engaged in almost all arm movements, so painful movement of the shoulder joint affects many everyday activities.

Frozen Shoulder is a long very painful condition of the shoulder, whereby it is completely or partially unmovable. Frozen shoulder often starts randomly but may be triggered by a mild injury to the shoulder or it has a common relation to hormonal imbalances or diabetes. The condition usually goes through 3 phases, starting with 1) pain, then 2) stiffness and finally 3) thawing, whereby the shoulder goes through a stage of resolution as the pain eases and most of the movement returns slowly. This process may take a long time, sometimes as long as two or more years.

AC joint pain is associated with pain and tenderness at the top of the shoulder and difficulty reaching the arm across the body, like putting on a seat belt, for example. The AC joint is located at the top of the shoulder where the collarbone joins with the shoulder blade. This joint deals with high compressive forces and can also be associated with arthritis.

TENNIS ELBOW - EPICONDYLITIS

 ELBOW

Tennis elbow is a common condition where you have pain on the outer side of the elbow where the muscle tendons attach onto the bone. It is commonly caused by overuse and repetition, causing damage to tendons. The medical term for tennis elbow is 'lateral epicondylitis'. For most people with tennis elbow, the pain only occurs when they use their forearm and wrist, particularly for twisting movements such as using a screwdriver or opening a jar. However, for some people, the pain is constant and can even affect sleep. Tennis elbow has also been associated with occupations that require vibration tool or even drivers.

PLANTER FASCIITIS

Plantar fasciitis is the most common form of heel pain. Runners and athletes with plantar fasciitis may notice heel pain that gets progressively worse during a workout. Left untreated over weeks or months, the pain may become worse or more frequent, interrupting daily activities. In plantar fasciitis, fibrous connective tissue at the undersurface of our foot becomes inflamed over time. It begins at the heel bone and widens as it spreads out across the bottom of the foot.
